The document below is a page of an evaluation I performed at the skilled nursing facility. All forms of identification have been omitted. In the evaluation process, I established my clients' meaningful occupations, client factors, performance patterns, performance skills, and the environment/context that they planned to go back to. To get a better idea about their roles and routines I asked them what a typical day looked like for them. After that, I would have them perform a few assessments, such as range of motion and manual muscle testing to determine what their potential physical barriers were. Finally, I would have my clients perform a few ADLs to see what they needed to work on in order to engage in their every day occupations.
With this particular client in the evaluation below, my goals and treatment sessions mainly focused on self-care tasks, as she needed assistance with the majority of her ADLs. We would go through her typical morning routine every day, and I tried to make the environment as natural as possible. We would then go to the therapy gym to work on upper body strengthening exercises and cognitive tasks, such as locating and sorting grocery items in the cabinet. After that, we progressed to IADL tasks, such as cooking and cleaning in the kitchen and ADL suite at the facility. After a few months, I was able to see significant improvements with this client, and she was able to return home only requiring minimal assistance with IADLs.
